If you have Namira's Shroud equiped while talking to Varos and lending him the item during the Namira's Shroud quest, the item will be removed from your inventory, but the script associated with it persists.

(I haven't tested if the fact that I restored personality had anything to do with the bug though. I doon't think so, but if there's trouble replicating the bug, maybe that could be why.)
