Inside the locked bedroom under the stairs in the Daranith & Daranith: Barristers interior in remade Firewatch, there's a gap in the wall looking out into the hallway. From the hallway, the gap is invisible.
The whole town of Uman, in one of the most picturesque locations of DoD, lacks any and all dialogue. Just a couple of lines in e.g., "Uman", "Services", "Specific place", "latest rumors", etc, would really help this place.
The St. Felms monastery, when it still used to be called Iliath, boasted a "forbidden library" in its tower. Now the library has been removed, but some of the dialogue still mentions it. Case in point -- tr_m4_Ralane Tholeno in the topic "little secrets".
In Gindaman's dialogue (Firewatch MG), it would be good to mention that the rank of Magister is required for his quests. Currently, when he tells you "I have no duties for %PCRank", you have no idea when to come back, unlike all vanilla questgivers who always tell you which rank is required for their quests. Also, it might be a good idea to add one more journal id before The Tower of The Enchantress quest, so it won't appear in journal until you reach the rank of Magister and ask Gindaman about duties.
In the quest TR_m2_TMo_GoneToGround in Tel Mothrivra, the script on TR_m2_Staron that is supposed to move him to Helnim still points to the cell Cephorad Keep, Interior. As a result, he will stay in the inn and act like he teleported out when you talk to him again.
And in a minor issue, if you already have the topic "scroll" (which you get from a vanilla Hlaalu quest), you can bypass some of Staron's dialogue, making him ask you for a scroll without telling you why he needs one. This doesn't break the quest, though.
The guards in the Packing Room of the Vauntus Mine (spelling may be incorrect - it's the diamond mine right outside Firewatch) seem to have deprecated weapons with missing assets. How did I come to discover this? It definitely wasn't due to stealing diamonds and getting caught.
